Complete the form called, "Registration packet October 2024 QME Exam":

Complete the form called, "Application for Appointment as Qualified Medical Evaluator, QME Form 100" AND attach the required documents.

Write a $125 check to: "Division of Workers' Compensation"
Mail the three above items (1. Registration form, 2. Application form + required documents, and 3. $125 check) to:

Once your registration packet has been processed, you will receive a notification, usually via email, to select a date to take the examination at a Pearson-VUE testing center. Be sure to choose a date and time! Be sure it's confirmed!
Pass the QME Competency Examination (the exam is only offered twice per year).
Complete a 12-hour writing course (contrary to popular belief, you do NOT need to have completed a 12-hour writing course prior to taking the QME Competency Examination. The 12-hour course can be completed after the QME Competency Examination).
Approximately 45 days after you take the QME Competency Examination, you'll receive notification in the mail, or via email, that you passed! You then need to complete the enclosed QME appointment paperwork and return it to the DWC.
